Catchment Area (Practice Boundary)
You can search for your postcode by clicking the magnifier in the top right corner of the map, this will indicate whether you live within the boundary or not.Named GP
We have allocated a Named Accountable GP for all of our registered patients.
Whilst we do not operate a system of only seeing your named GP, if you wish for your named GP to be changed, reasonable efforts will be made by the practice to accommodate this request.
If you do not know who your named GP is, please ask a member of our practice staff/ health navigators who will be able to inform you.
Access to Health Records
Should you require copies of your medical records please download and complete the form below. Once you have completed the form you can return the form to the surgery by the following methods:
- Post
- Drop the form into the surgery via reception
- Hand post into the surgery post box. (The post box is situated at the front gates)
- Email to lennardsurgery@nhs.net
