At The Lennard Surgery, our clinicians are unable to witness or sign Lasting Power of Attorney (LPA) or (POA) forms.
Who Can Be a Certificate Provider for My LPA?
A Certificate Provider is someone who confirms that you understand what you are doing when making a Lasting Power of Attorney (LPA) and that you are not under pressure to do so.
A Certificate Provider must:
- Not be related to the donor or attorney(s)
- Be aged 18 or over
- Have known the donor well for at least 2 years
- Not be an employee of the donor or attorney(s)
They can also be one of the following professionals:
- A solicitor, barrister, or advocate
- A registered social worker
- An independent mental capacity advocate (IMCA)
